Use this form to apply for benefit for the first time, or if you have had a break in your claim
Change of Address Form
if you are receiving benefit and you change address please use this form to let us know
Starting Work Form
if you are receiving benefit and you start work please use this form to let us know
Overlap Form (pdf, 27 KB)
You can usually only claim benefit on one home. However, if you have to pay rent on two homes you may be able to claim benefit for up to four weeks.
Backdate Form
If you have submitted a late claim for Housing and/or Council Tax Benefit and you think your claim should be backdated, you will need to complete this form.
Appeal Form (pdf, 19 KB)
Please complete this form if you believe your decision is wrong. You must also give the reason why you believe your benefit has been assessed incorrectly.
Application for Discretionary Housing Payment
If your rent has been restricted then you can complete this form if you wish to claim Discretionary Housing Payment
Certificate of Earnings
Use this form if you do not have 5 weekly or 2 monthly payslips
Certificate of Estimated Earnings
If you have just started work and do not have any payslips you can ask your employer to complete this form
Proof of Rent
If you do not have proof of your rent you can ask your landlord to complete this form
Self Employed Earnings Form
If you have recently started self employment, or you do not have trading accounts you can use this form to provide the information we need.
Application for Payment direct to Landlord
If you think that your benefit should be paid directly to your landlord please complete this form and we will consider your application.
